As you begin to have employees who will use the paid leave and enhanced FMLA time available, please remember that we need a COVID leave form filled out for each person requesting these types of leave.  If you need a copy of the form, please email us at hr@integrityhrm.com.

When turning in time, use the following codes to pay people for these leaves:
COVID19FMLA:  Paid enhanced FMLA leave time
COVID19SICK:    Paid sick leave time at 100% up to $511/day (reasons 1 through 3)
COVID19CARE:  Paid sick leave time at 2/3 regular rate (reasons 4-6)
